What is needed is to assess the condition first and foremost on the over-all characteristic pattern of symptoms, with loss of energy, enthusiasm, as well as reduced early morning erections,[link widoczny dla zalogowanych], erectile function,[link widoczny dla zalogowanych], and libido, together with depression, joint pains, weight gain and night sweats. If these symptoms are relieved by a properly monitored ‘therapeutic trial’ of testosterone treatment, then the diagnosis is confirmed. As a TV meerkat would say ‘Its Simple!’.
Dr Malcolm Carruthers is one of the very few UK based physicians who specialise in the diagnosis and treatment of testosterone deficiency. With a broad training in general and laboratory medicine, for the last twenty years he has specialised in Men’s Health, andrology, particularly treating the Andropause aka Male Menopause.
He is Medical Director of the Centre for Men’s Health. His consultations cover the investigation and treatment of the wide range of general medical problems which can cause testosterone deficiency, and ensure a balanced discussion of benefits and possible risks when testosterone treatment is being considered.
Dr Malcolm Carruthers, Centre for Men’s Health have researched and released articles on testosterone deficiency. More of his works can be found at
He has taught nationally and internationally on TRT (Testosterone Replacement Therapy),[link widoczny dla zalogowanych], and with three popular books on the subject, Male Menopause,[link widoczny dla zalogowanych], Maximising Manhood and The Testosterone Revolution, together with a textbook, ADAM : Androgen Deficiency in the Adult Male, is a widely recognised expert on the subject.
Because of Dr Carruthers evidence-based theory of ‘Testosterone Resistance’ published in the Journal of Sexual Medicine (see the testosterone level becomes largely irrelevant, just as would the level of insulin in an insulin resistant maturity onset diabetic. It is this insistence on basing the diagnosis on inaccurate and irrelevant blood tests which makes testosterone deficiency syndrome the most common hormonal disorder in the adult male, but the least often treated. The sad statistic is that only 1% of the 20% of men over the age of 50 likely to be suffering from testosterone deficiency get treatment. Time for a change?
A new 5 year study in the prestigious New England Journal of Medicine by the European Male Ageing Study Group has finally acknowledged that the male menopause or andropause, which they ponderously and incorrectly term ‘Late-Onset Male Hypogonadism’, actually exists. However, they consider it a rare condition which occurs in less than 2% of men over the age of 50.
This view contrasts with that of Dr Malcolm Carruthers who published evidence in the Aging Male Journal last year showing that it is far more common than generally realised. Like an iceberg, only the tip of this serious and sometimes life-threatening problem is revealed by lab tests, representing just 10% or less or less of the true condition as diagnosed by symptom questionnaires, such as the AMS scale which is available on-line at As the Titanic found, this is a serious over-sight, which is wrecking many men’s lives unnecessarily.
